Job summary

American Physiatry in North Carolina seeks a compassionate Nurse Practitioner to join our team in skilled nursing facilities, focusing on pain control, function, and rehabilitation. You will work closely with a physiatrist and rehabilitation staff to assess patients and manage caseload.

You will develop individualized care plans, educate patients and families, review medications, and monitor progress to achieve optimal outcomes. A North Carolina NP license and Master’s degree are required.

Qualifications

• Licensed Nurse Practitioner with North Carolina certification.
• Experience managing your own caseload.
• Submit accurate and thorough clinical documentation promptly, ensuring compliance with policies and regulatory requirements.

Responsibilities

• Conduct thorough assessments of patients under the supervision of a Physiatrist.
• Develop and implement individualized care plans to improve function and rehabilitation.
• Collaborate with the healthcare team on medication management and therapy recommendations.
• Educate patients and families on treatment plans, health maintenance, and preventive care.
• Monitor and evaluate patient progress and adjust care plans to achieve optimal outcomes.
