Job summary

Western Wayne Family Health Centers seeks a Certified Nurse Practitioner (CNP) to provide primary health care in outpatient settings as part of a collaborative care team. The CNP will work with physicians, conduct patient assessments, order and review tests, and coordinate follow-up with other health services.

The role requires MI NP licensure, CNP certification, BLS, and at least 3 years of outpatient primary care experience.

Qualifications

• Must be a Registered Nurse with Michigan NP licensure.
• Certification as a Michigan Certified Nurse Practitioner (CNP).
• Minimum 3 years outpatient primary care experience preferred; BLS certification required.

Responsibilities

• Deliver primary health care in outpatient settings.
• Collaborate with physicians in a team-based care model.
• Review lab results and medical records; coordinate follow-up.
• Document services accurately per QA plan and privacy policies.
