Job summary

Western Wayne Family Health Centers in Lincoln Park, MI is seeking a Certified Nurse Practitioner to deliver primary health care in an outpatient setting as part of a collaborative care team. The CNP will follow a physician collaborative agreement and provide health education, medication management, and timely documentation.

Ideal candidates hold Michigan NP licensure and CNP certification, with at least 3 years of outpatient experience, active BLS, and a commitment to community health.

Qualifications

• Must be a Registered Nurse.
• Must have State of Michigan licensure as a Nurse Practitioner.
• Certification in Michigan as a Certified Nurse Practitioner (CNP).
• Minimum 3 years outpatient primary care experience preferred.
• Maintains up to date BLS certification
• Must be able to meet standards required to be credential by appropriate insurance and regulatory plans.
• Must remain board certified by the AANP or ANCC.
• Must have permanent, unrestricted license to practice medicine in the State of Michigan.
• Flexibility in days and hours available for scheduled work.
• Experience working in a public health setting.

Responsibilities

• Delivers primary health care to designated outpatient consumers, collaborating with medical providers.
• Works with collaborating physician per agreement.
• Records keeping including follow-up of missed appointments and tracking of tests.
• Provides consultation and health education.
• Dispenses medications as licensed.
• Renders first aid when needed.
• Advocates for client interests and coordinates follow-up with providers.
• Maintains confidentiality and privacy.
• Participates in committee work and quality improvement activities.
