HCRC consulting is seeking a compassionate NP/CRNP or PA to join our Towanda, PA practice delivering primary and chronic care management in long-term care communities. We offer a flexible full-time or part-time position with a total compensation package.

Qualifications

• Current or eligible Pennsylvania Nurse Practitioner or Physician Assistant license.
• Current DEA registration or ability to obtain.
• Experience in geriatrics, long-term care, skilled nursing, family medicine, internal medicine, or primary care is preferred but not required.

Responsibilities

• Provide primary and chronic care management services to residents in long-term care communities.
• Deliver care in Skilled Nursing, Assisted Living, and Independent Living settings.
• Collaborate with interdisciplinary team to coordinate patient care and ensure quality outcomes.

We are seeking a compassionate and motivated Nurse Practitioner (NP/CRNP) or Physician Assistant (PA) to join our growing practice in Towanda, Pennsylvania. Our providers deliver comprehensive medical care to residents in Skilled Nursing, Assisted Living, and Independent Living communities. As a Nurse Practitioner or Physician Assistant, you will provide primary and chronic care management services to residents in long-term care communities.

Each provider receives support from a dedicated Regional Nurse Coordinator who assists with:

• Daily scheduling and care coordination
• Documentation and note management
• Facility communication
• Prescription refills and medication requests
• Forms, orders, and administrative tasks
• Communication with nursing staff, families, pharmacies, and other healthcare partners

Compensation

• Up to $125,000 annually with Total Compensation Package: $133,000–$150,000+ annually
