Job summary

Senior PsychCare (SPC) is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to join our mission-driven behavioral health team serving nursing home and assisted living residents in San Augustine, TX. You will perform cognitive assessments, manage psychotropic medications, and collaborate with nursing staff as part of SPC’s multidisciplinary team.

Qualifications

• Licensed NP or PA in Texas required.
• Geriatric mental health experience preferred.
• Spanish bilingual is a plus.

Responsibilities

• Conduct cognitive and emotional assessments to set/manage treatment plans.
• Manage psychotropic medication and gradual dose reductions.
• Collaborate with nursing home staff and SPC’s behavioral health team.
• Participate in treatment team meetings (in-person and virtual).
• Deliver care both in-person and via telehealth when appropriate.
• Maintain accurate, timely documentation in the EHR system.

Qualifications

• Licensed NP or PA in the state of Texas
• Psychiatric/mental health experience required
• Experience with geriatric populations strongly preferred
• Spanish bilingual is a plus
• Must have a valid driver’s license and be comfortable traveling locally
