Senior PsychCare in Holdenville, OK is seeking a Nurse Practitioner or Physician Assistant to join a collaborative, mission-driven mental health team serving nursing home and assisted living residents. The role offers full-time or part-time hours with clinical support from psychiatrists, telehealth options, and a focus on continuity of care.

You’ll conduct cognitive and emotional assessments, manage psychotropic medications, and document in the EHR.

Qualifications

• Licensed NP or PA in Oklahoma
• Psychiatric/mental health experience required
• Geriatric population experience preferred
• Spanish bilingual is a plus
• Valid driver's license and local travel
• Adaptive, flexible, team-based care

Responsibilities

• Conduct cognitive and emotional assessments to set/manage treatment plans
• Manage psychotropic medications and gradual dose reductions
• Collaborate with nursing home staff and SPC's behavioral health team
• Participate in treatment team meetings (in-person and virtual)
• Deliver care both in-person and via telehealth when appropriate
• Maintain accurate, timely documentation in our EHR system

Qualifications
