Job summary

AccessHealth seeks a qualified NP/PA to perform comprehensive in-home health assessments for adults and seniors, reviewing medical history, medications, social factors, and risks to improve care quality.

Provider conducts up to one-hour assessments at members’ residences or SNFs, orders tests, and coordinates with physicians and care teams. Flexible scheduling and ongoing professional support accompany competitive compensation.

Qualifications

• Master's degree or equivalent with NP/PA licensure.
• Board certification in Adult, Geriatrics, Family, Emergency, or Acute Care.
• Current BLS/ACLS/CPR certification.
• 1+ year NP/PA experience preferred; new grads encouraged.
• Valid driver’s license, automobile insurance; strong computer skills and communication.

• Administer and order point-of-care and lab tests.

Job description

Key Responsibilities

• Perform assessments lasting up to one hour at members' residences, including health education, diagnosis, treatment, referrals, and documentation.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, family members, and other healthcare professionals.
• Support patient education and follow-up in coordination with primary care providers.
• Work in various settings such as homes or Skilled Nursing Facilities.

Qualifications & Skills

• Master's Degree or equivalent experience with NP or PA licensure, current licensure with prescriptive authority.
• Board certification in relevant specialty (Adult, Geriatrics, Family, Emergency, or Acute Care).
• Current BLS, ACLS, or CPR certification.
• At least one year of NP or PA experience preferred; new grads encouraged.
• Valid driver's license, automobile insurance, strong computer skills, and excellent communication abilities.
