A community health center in Kansas City is hiring a Nurse Practitioner or Physician Assistant to provide primary care using the Patient Centered Medical Home model. You'd deliver acute, chronic, and preventive care with a particular emphasis on chronic disease management and pediatrics, working under the direction of a collaborating physician.

Key Responsibilities.

• Provide diagnostic and therapeutic services, including history and physical exams, prescribing, and management of acute, chronic, and preventive care under a collaborating physician.
• Make referrals for hospital and specialty services where appropriate.
• Review and interpret lab, radiology, and consultation reports promptly, directing nursing staff or contacting patients directly with results and treatment recommendations.
• Deliver care aligned with current evidence-based guidelines and state, local, and federal requirements.
• Provide ongoing patient education on diagnosis and plan of care.
• Help establish standards of care in collaboration with other providers and staff.
• Document thoroughly — reason for visit, medical history, current medications, plan of care, orders, referrals, transitions of care, and follow-up.
• Identify barriers to care and coordinate referrals to internal support services and outside collaborating organizations.
• Collaborate with the Chief Medical Officer, Chief Clinical Officer, and other providers to evaluate services and develop programs.
• Participate in monthly Medical Executive meetings, committee meetings, quality improvement activities, peer review, and pharmacy and therapeutics.
• Stay current with advances in therapeutic intervention through ongoing literature review.
• Maintain continuing education requirements and patient confidentiality in accordance with HIPAA.

Qualifications.

• Active, unrestricted Missouri FNP or PA license, in hand at time of application — this is a hard requirement, and license-eligible or in-process candidates will not be considered.
