GENERAL OVERVIEW

This position is an integral part of the care delivery system, working to provide a healing environment and placing our customers first. The incumbent provides primary healthcare services for non‑acute or chronic health conditions in specific populations, in accordance with applicable scope and standards of practice and with the policies, values, and mission of the organization within the CRNP scope of practice. Work is performed in a physician office or offices dedicated to Primary Care, with weekend and on‑call coverage on a rotational basis, as required.

Schedule: Monday‑am/pm/eve, Friday‑am/pm/eve, Sat 8am‑2pm.

ESSENTIAL RESPONSIBILITIES

• Provides appropriate patient care, including recording patient medical history and physical assessment, developing therapeutic care plans, ordering diagnostic tests, interpreting test results, and providing accurate medical record documentation. 25%
• Communicates and coordinates patient care with patient, family, community health services, and interdisciplinary team. Involves attending physician regarding plan of care and clinically decides on issues outside the CRNP's knowledge base or scope of practice. 15%
• Provides monitoring and continuity of care and coordinates periodic evaluation by physicians as defined in the written agreement between the CRNP and the supervising physician(s). 10%
• Prepares written prescription orders for drugs and controlled substances, administers medication and injections, sutures wounds, and provides wound care. 10%
• Educates patients and family members regarding health and preventive medicine, medications prescribed, treatment instructions, and recommends community resources as appropriate. 10%
• Triages patient telephone calls, provides monitoring and continuity of care. 10%
• Participates in, develops, and implements performance and/or quality improvement initiatives. Ensures compliance with all regulatory agencies. 10%
• May perform other duties such as managing outpatient concerns and any other duty specific to specialty. 10%
• Other duties as assigned or requested.

QUALIFICATIONS

Required

• Master's degree (MSN) from a State Board of Nursing approved Master’s Program.
• National Board Certification for Nurse Practitioners through AANP, ANCC, NCC, or PNCB.
• Practicing‑state licensure as a Nurse Practitioner.
• Prescriptive Authority from the practicing state within 60 days of hire.
• Drug Enforcement Agency licensure within 60 days of hire.
• CPR certification – American Heart Association.
• Act 34 Criminal Background Clearance Certificate.
• Act 33 Child Abuse Clearance Certificate.
• Act 73 FBI Fingerprinting Criminal Background Clearance Certificate.

VARIABILITY / SUBSTITUTIONS

• Four to ten years related experience and/or training; or equivalent combination of education and experience.
