General Overview

In addition to your base compensation, this role offers the exciting opportunity to earn an annual quality-based bonus. This bonus is directly tied to your contributions to our population health initiatives and value-based metrics, rewarding your dedication to a team‑based approach, improving patient outcomes and driving efficient, high‑quality care. This job is an integral part of the care delivery system working together to provide a healing environment by placing our customers first. The incumbent provides primary healthcare services for non‑acute or chronic health conditions in specific populations in accordance with applicable scope and standards of practice and with the policies, values, and mission of the organization within the CRNP scope of practice. Work is performed in a physician office or offices dedicated to Primary Care, with weekend and on‑call coverage on a rotational basis, as required.

Schedule: Tuesday‑Friday evenings and Saturday 8 am‑2 pm.

Essential Responsibilities
  • Provides appropriate patient care, including recording medical history and physical assessment, developing therapeutic care plans, ordering diagnostic tests, interpreting results, and documenting accurately. (25%)
  • Communicates and coordinates patient care with patients, families, community health services, and interdisciplinary teams; involves attending physicians in clinical decision‑making for issues outside the CRNP’s knowledge base or scope of practice. (15%)
  • Monitors continuity of care and coordinates periodic evaluation by physicians as defined in the written agreement between the CRNP and supervising physician(s). (10%)
  • Prepares written prescription orders for drugs and controlled substances, administers medication and injections, sutures wounds, and provides wound care. (10%)
  • Educates patients and family members regarding health, preventive medicine, medications, treatment instructions, and recommends community resources as appropriate. (10%)
  • Triages patient telephone calls and provides monitoring and continuity of care. (10%)
  • Participates in, develops, and implements performance and/or quality improvement initiatives; ensures compliance with all regulatory agencies. (10%)
  • May perform other duties such as managing outpatient concerns and any other duty specific to the specialty. (10%)
  • Other duties as assigned or requested.
Required Qualifications – Nurse Practitioner
  • Master's degree (MSN) from an approved Master’s Program.
  • National Board Certification for Nurse Practitioners (AANP, ANCC, NCC, or PNCB).
  • License to practice as a Nurse Practitioner in the state.
  • Prescriptive authority from the state within 60 days of hire.
  • Drug Enforcement Agency licensure within 60 days of hire.
  • CPR certification (American Heart Association).
  • Act 34 Criminal Background Clearance Certificate.
  • Act 33 Child Abuse Clearance Certificate.
  • Act 73 FBI Fingerprinting Criminal Background Clearance Certificate.
Required Qualifications – Physician Assistant
  • Master’s degree in Physician Assistant Studies.
  • Current PA Physician Assistant license.
  • PA‑C or board‑eligible; ongoing successful recertification (NCCPA) as a PA‑C within 90 days of hire.
  • Drug Enforcement Agency licensure within 60 days of hire.
  • CPR certification (American Heart Association).
  • Act 33 Child Abuse Clearance Certificate.
  • Act 34 Criminal Background Clearance Certificate.
  • Act 73 FBI Fingerprinting Criminal Background Clearance Certificate.
Substitutions
  • Four to ten years of related experience and/or training; or an equivalent combination of education and experience.
