Job summary

Qualifications

  • Active Connecticut APRN or Physician Assistant license.
  • Graduate of an accredited Nurse Practitioner or Physician Assistant program.
  • DEA and Connecticut controlled-substance registration, or eligibility to obtain both.

Responsibilities

  • Provide comprehensive primary and ambulatory care to a diverse patient population.
  • Obtain medical histories, perform assessments, diagnose conditions, and interpret clinical findings.
  • Develop and manage evidence-based treatment plans in accordance with clinical standards.
  • Educate patients and collaborate with physicians, nurses, behavioral health professionals, and other care team members.
  • Maintain timely, accurate, and thorough clinical documentation within the electronic health record.
  • Support organizational quality measures and continuity-of-care initiatives.
