Join our team as a Nurse Practitioner or Physician Assistant at Optum, providing in-home assessments across Utah. This role offers flexible scheduling, no on-call, weekends, or holidays, and involves travel across the state, with all expenses covered.

Key Responsibilities:

1. Conduct comprehensive in-home assessments, including medical history, physical exams, and vital signs.
2. Perform evidence-based screenings and point of care testing as appropriate.
3. Identify diagnoses and formulate care plans based on assessments.
4. Communicate findings to primary care providers and identify urgent situations for intervention.
5. Educate patients on health topics, medication adherence, and disease management.

Qualifications:

• Active NP or PA license or ability to obtain one by start date, free of restrictions.
• Active national certification (e.g., ANCC, AANP, NCCPA) or ability to obtain it.
• Physical ability to perform assessments, including lifting and navigating various home environments.
• Reliable transportation for travel up to 85% within Utah.

Preferred Experience:

• 1+ years in clinical practice.
• Experience in geriatrics, cardiology, internal medicine, or endocrinology.
• Home health or home visit experience.

The salary ranges from $89,800 to $176,700 annually, plus benefits, incentives, and coverage of travel expenses. This role is ideal for those seeking work-life balance and meaningful impact in patient care.
