This position offers $40,000 Student Loan Repayment or a $25,000 Sign-on Bonus for eligible individuals. Optum seeks a Nurse Practitioner or Physician Assistant to join our HouseCalls team in Utah, providing annual in-home assessments to promote preventive care and improve patient health outcomes.

As part of our Home and Community Care team, you'll deliver high-quality medical services, addressing physical, mental, and social needs across the state. Enjoy flexible scheduling with no on-call, weekends, or holidays, and travel approximately 85% of the time across Utah, with all travel expenses covered.

Primary Responsibilities

• Conduct in-home assessments: medical history, review of systems, medication reconciliation, vital signs, physical exams
• Perform screenings and point of care testing as appropriate
• Identify diagnoses for care management and treatment
• Communicate findings to inform care and identify care gaps
• Identify urgent/emergent issues and intervene appropriately
• Educate members on disease management, medications, and compliance

Minimum Qualifications

• Active NP or PA license, or ability to obtain by start date; free of disciplinary actions
• Active national NP or PA certification or ability to obtain it in Utah by start date
• Physical ability to perform job duties (lifting, navigating stairs, physical assessments)
• Reliable transportation to travel within Utah

Preferred Qualifications

• 1+ years of clinical experience in relevant settings
• Experience in Gerontology, Cardiology, Internal Medicine, or Endocrinology
• Home health or home visit experience

The salary range is $89,800 to $176,700 annually, based on experience and location, with comprehensive benefits and incentives. Employees must remain in the role for at least 12 months; full benefits of student loan repayment are contingent on 36 months of employment in this role.
