Job Summary:

Geisinger is seeking an experienced Nurse Practitioner or Physician Assistant to join our 65 Forward Primary Care team as a virtual, remote provider. Provider must reside in state of PA and be licensed in the state of PA.

Job Duties:

• 30-minute appointments for all visits
• Provides acute access management to support physician panels
• Daily huddles with care team members to review coordination of care
• Pharmacists to assist with patient education and medication management

EDUCATION/EXPERIENCE:

Advanced Practitioner must:

• Have 5+ years of experience or more in Geriatric and/or Primary Care experience, Hospitalist experience preferred
• Reside in the state of PA
• Private workspace

The successful Physician Assistant candidate will work under the responsibility and supervision of the physician and require completion of an ARC-PA (or predecessor organization)approved physician assistant program, current NCCPAcertification or eligibility and Pennsylvania State licensure.

The successful Nurse Practitioner candidate will work in collaboration with the physician and have a certificate of completion from an approved program for Nurse Practitioners or be eligible to hold national certification that allows the NP to work with the patient populations and location settings associated with this position. Must be eligible for licensure as a nurse practitioner in the state of PA.
