Job summary

Atlas Medical is expanding its Home Health & Hospice Provider Team. We seek a Nurse Practitioner or Physician Assistant to deliver mobile, on-site primary care to vulnerable seniors in Scottsdale, AZ.

The role requires autonomous management of complex cases, education for patients and families, and close collaboration with facility staff and Atlas teams. Minimum 1 year of primary care experience and valid AZ NP/PA licensure are required.

Qualifications

• Must have at least one (1) year of primary care experience.
• Valid Licensure as a Nurse Practitioner or Physician Assistant in State of Practice.
• Ability & Desire to work independently, and drive between communities.
• We currently use Elation as our EMR.
• Effective verbal & written communication & time management skills.
• Valid AZ Fingerprint Clearance Card.
• Current CPR Certification & Proof of Negative TB Test.

Responsibilities

• Deliver mobile on-site primary care services to residents in assisted living and senior communities.
• Autonomously manage complex healthcare scenarios and educate patients and families on self-care and disease management.
• Collaborate daily with facility staff and Atlas Patient Care support teams.
• Uphold confidentiality and follow Atlas quality measures for high-standard care.

Job Summary

As a Medical Provider for our Atlas Medical Team, you will be entrusted with delivering mobile on-site primary care services to our vulnerable patient population residing in assisted living and other senior communities. Your expertise will extend beyond medical treatment, as you educate patients and their families on self-care, disease management, and end-of-life care. Collaboration will be a daily aspect of your work, liaising closely with facility staff and Atlas Patient Care support teams. All activities will be guided by quality measures provided by Atlas, emphasizing our commitment to high standards of care.
