Job summary

Atlas Medical is expanding its Home Health & Hospice Provider Team in Scottsdale, AZ, and seeks a dynamic Nurse Practitioner or Physician Assistant to deliver mobile on-site primary care to vulnerable seniors in assisted living communities.

You will work autonomously, manage complex cases, educate patients and families on self-care and disease management, and collaborate with facility staff and Atlas Patient Care teams while upholding confidentiality and Atlas standards.

Qualifications

• Must have at least one year of primary care experience.
• Valid Licensure as a Nurse Practitioner or Physician Assistant in state of practice.
• Ability and desire to work independently and drive between communities.
• We currently use Elation as our EMR.
• Effective verbal and written communication and time management skills.
• Valid AZ Fingerprint Clearance Card.
• Current CPR Certification and proof of negative TB test.

Responsibilities

• Deliver mobile on-site primary care to vulnerable seniors in assisted living and similar communities.
• Autonomously manage complex healthcare scenarios with multiple comorbidities.
• Educate patients and families on self-care, disease management and end-of-life care.
• Collaborate daily with facility staff and Atlas Patient Care support teams.
• Uphold Atlas quality measures and maintain patient confidentiality.

Job Summary
