Job summary

Atlas Medical is expanding our Home Health & Hospice Provider Team in Arizona and seeks a Nurse Practitioner or Physician Assistant to deliver mobile on-site primary care to vulnerable seniors in assisted living and similar communities.

The role requires autonomous management of complex health issues, patient and family education on self-care, disease management, and end-of-life care. Collaboration with facility staff and Atlas Care teams is essential, while confidentiality is upheld.

Qualifications

• At least 1 year of primary care experience.
• Valid licensure as a Nurse Practitioner or Physician Assistant in state of practice.
• Ability and desire to work independently, and drive between communities.
• We currently use Elation as our EMR.
• Effective verbal and written communication and time management skills.
• Valid AZ Fingerprint Clearance Card.
• Current CPR Certification and proof of Negative TB Test.

Responsibilities

• Deliver mobile on-site primary care services to residents in assisted living and other senior communities.
• Autonomously manage complex healthcare scenarios with multiple co-morbid conditions.
