Job summary

Alteas Health is seeking a Nurse Practitioner or Physician Assistant to deliver specialty Cardiology and Pulmonology care in Skilled Nursing, Long-Term Care, and Post-Acute Care settings across Columbia, IL and surrounding communities.

You will work with physicians and interdisciplinary teams to provide high-quality, patient-centered care, with daily travel within the assigned territory and a Monday–Friday daytime schedule.

Qualifications

• Current NP or PA license in Illinois
• National certification appropriate to licensure
• Current BLS certification

Responsibilities

• Evaluate, diagnose, and manage cardiovascular or pulmonary conditions.
• Develop individualized treatment plans using evidence-based practice.
• Order, interpret, and communicate labs and imaging results.
• Prescribe medications within scope of practice.
• Educate patients and families about diagnoses and care plans.
• Coordinate care with physicians, nursing staff, and other professionals.
• Maintain timely, accurate clinical documentation.
• Round at Skilled Nursing and Long-Term Care facilities.
• Build relationships with facility leadership and teams.

Assigned Territory

This position serves patients throughout the Columbia, IL area, providing specialty care at Skilled Nursing, Long-Term Care, and Post-Acute Care facilities located in Columbia, O\'Fallon, Edwardsville, and surrounding communities. Daily travel within the assigned territory is expected to support patient care and operational needs.

Providers enjoy a full-time, Monday through Friday daytime schedule with no regularly scheduled weekends or traditional on-call responsibilities, allowing for an exceptional work-life balance.

What You\'ll Do

As a Nurse Practitioner or Physician Assistant with Alteas Health, you\'ll provide specialty Cardiology services while working collaboratively with physicians, facility staff, and interdisciplinary care teams to deliver high-quality, patient-centered care.

Responsibilities Include

• Evaluate, diagnose, and manage acute, chronic, and episodic cardiovascular or pulmonary conditions.
• Develop individualized treatment plans using evidence-based clinical practice.
• Order, interpret, and communicate laboratory studies, imaging, and other diagnostic testing.
• Prescribe medications and treatments within your scope of practice.
• Educate patients, families, and caregivers regarding diagnoses, treatment plans, medications, and ongoing care.
• Coordinate care with physicians, nursing staff, therapy, pharmacy, social services, and other healthcare professionals.
• Maintain accurate, timely, and compliant clinical documentation.
• Round at assigned Skilled Nursing, Long-Term Care, and Post-Acute Care facilities.
• Build strong relationships with facility leadership and clinical teams.
• Represent Alteas Health with professionalism, integrity, compassion, and clinical excellence.

Required Qualifications

What We\'re Looking For

• Current, unrestricted Nurse Practitioner or Physician Assistant license in the applicable state(s) of practice.
• Current national certification appropriate to licensure.
• Current Basic Life Support (BLS) certification.
• Minimum six (6) months of Nurse Practitioner or Physician Assistant clinical experience.
• Valid driver\'s license with reliable transportation.
• Ability to travel daily throughout an assigned territory.
• Strong clinical judgment, communication, organizational, and critical thinking skills.
• Experience utilizing Electronic Medical Records (EMR).

Preferred Qualifications

• Two (2) or more years of Nurse Practitioner or Physician Assistant experience.
• Experience in Skilled Nursing, Long-Term Care, Post-Acute Care, Internal Medicine, Cardiology, Pulmonology, Hospital Medicine, or Geriatrics.
• Experience caring for medically complex patient populations.
