Job summary

Gilder Search Group is seeking a Nurse Practitioner or Physician Assistant for a Skilled Nursing Facility (SNF) in Chicago, IL. This full-time, in-person role offers a Monday-Friday schedule, 9 to 5, with no nights, no weekends and no on-call.

The last two hours of the day can be completed remotely. You will provide primary care, manage a panel of residents, order labs and collaborate with an interdisciplinary team while maintaining compliant documentation.

Qualifications

• Active Illinois NP or PA license with DEA registration.
• Board certification as FNP/AGNP/AGPCNP, or NCCPA for PA.
• Experience in skilled nursing or geriatrics preferred.
• Ability to manage a full resident panel independently.

Responsibilities

• Provide primary care to SNF residents including exams and medication management.
• Diagnose and treat chronic and acute conditions within scope.
• Order and interpret labs and diagnostics and adjust care plans accordingly.
• Collaborate with facility nursing, therapy and the interdisciplinary team.
• Maintain accurate clinical documentation.
