Horizon Health in Wessington Springs, SD is seeking two full-time Nurse Practitioner or Physician Assistant to provide direct patient care for all patients. ER and hospital coverage is required with weekday and weekend rotation; ACLS/ATLS/PALS certifications or willingness to obtain them upon hire.

Qualifications

• Licensed as NP or PA in South Dakota.
• ACLS, ATLS and PALS certifications or willing to obtain.
• Experience in primary care and care coordination.
• Ability to lead care teams and provide direct patient care.

Responsibilities

• Provides primary health care to patients following evidence-based guidelines.
• Leads care teams to deliver high-quality healthcare per improvement plans.
• Manages long-term care, develops care and self-management plans.
