Horizon Health in Wessington Springs, SD is seeking a full-time Nurse Practitioner or Physician Assistant to provide direct patient care for all patients. The organization is looking for a highly professional individual with strong communication skills who is committed to coordinating all aspects of primary care for the patients served.

Qualificações

• Must hold an NP or PA license and be licensed to practice in South Dakota.
• ACLS, ATLS, and PALS certifications are required or must be obtained.
• Strong communication and team leadership skills with focus on patient-centered care.

Responsabilidades

• Provides primary health care to patients per evidence-based guidelines.
• Leads care teams to deliver high-quality healthcare per Horizon Quality Improvement Plan.
• Manages long-term care for acute and chronically ill patients, including care coordination and self-management plans.
