Elevate MD is growing—and we’re looking for a passionate, patient-centered Nurse Practitioner or Medical Doctor to join our provider team focused on hormone optimization, functional wellness, and longevity medicine.

What You’ll Do

• Evaluate and treat patients with symptoms of hormonal imbalance (low energy, weight gain, sleep issues, mood changes, libido concerns, etc.)

• Develop personalized treatment plans using bioidentical hormone replacement therapy (BHRT) and lifestyle guidance

• Review labs and diagnostics with a functional lens to uncover root causes

• Collaborate with our team of wellness coordinators, nutrition coaches, and cellular health specialists

• Educate and empower patients to take control of their health journey

What We’re Looking For

• Licensed NP or MD/DO in the state of Ohio (or ability to transfer licensure)

• Experience or interest in BHRT, functional medicine, and longevity care

• Strong interpersonal skills and a desire to build lasting patient relationships

• Open to ongoing training and collaboration

• Team player who thrives in a fast-growing, mission-driven environment
