Job Description

We're seeking a Remote Full-Time Nurse Practitioner or Physician Assistant with Salud Family Health supporting our clinic in Sterling, CO. This position will work approximately 70% Telehealth and 30% Inbox (administrative work -management). There is no direct in-person patient contact associated with this job description. The Remote APP approaches his/her tasks in a team-based care fashion that in addition to clinic duties, also supports specific providers by helping address the daily accumulation of work within Salud’s EMR software system, eClinicalWorks (eCW). Adheres to the spirit of the SALUD mission statement while performing assigned duties.

  • Conducts comprehensive assessment via Telehealth (video and/or audio), including history taking, review of systems, and evaluation of patient data
  • Diagnose and manage acute and chronic conditions within the APP scope of practice and established protocols and guidelines
  • Develop, implement, and monitor individualized treatment plans, including prescribing medications, ordering and interpreting diagnostic tests, and providing patient education.
  • Document patient encounters thoroughly and accurately in the Electronic Health Records (EHR) in a timely manner
  • Provide compassionate, patient-centered care through virtual channels, ensuring clear communication and rapport building
  • Identify patients requiring in-person care and coordinate appropriate follow-up
  • Have exceptional written and verbal communication skills in order to deliver medical information clearly and emphatically
  • Participate in team meetings, case reviews, and clinical quality improvement initiatives
  • Works in conjunction with other Medical Providers, Dental Providers, Behavioral Health Providers, and all other staff to ensure that the patients obtain the highest standards of integrated healthcare
  • Ability to recognize limitations and seek help when necessary
  • Keeps abreast of current evidence-based clinical guidelines, standards of care, and standards of practice in the provision of health center services
  • Performs other duties as assigned

Bubble’/Inbox Management (30%)

  • Efficiently and proactively manage the administrative workload generated with eCW’s ‘bubbles’ assigned to themselves and for specific physicians and APPs who are either out on leave or have left Salud without a replacement being yet hired. The specific physicians and APPs will be identified to the Remote APP by the Chief Medical Officer. Triage and respond to a wide range of patient messages, including but not limited to:
  • Symptom enquiry and advice requests
  • Medication refill requests
  • Lab/test result communication, interpretation, and follow-up plan
  • Post-visit follow-up questions
  • Referral follow-up and questions
  • Document review and signature Exercise independent clinical judgement within established protocols to:
  • Address patient concerns
  • Determine the need for a Telehealth visit, in-person visit, or other intervention
  • Renew prescriptions based on set protocols
  • Ensure timely response as established by protocols
  • Maintain meticulous documentation of all inbox interactions, decisions made, and actions within the EHR
  • Works in conjunction with other Medical Providers, Dental Providers, Behavioral Health Providers, and all other staff to ensure that patients receive the highest standards of integrated healthcare
  • Identify opportunities for process improvement
  • Ability to work independently, recognize limitations, and seek help when necessary or know when to escalate appropriately
  • Keeps abreast of current evidence-based clinical guidelines, standards of care, and standards of practice in the provision of health center services
  • Performs other duties as assigned

Qualifications

Education:

  • Completion of a certified Physician Assistant program with state and national certification as evidenced by a certificate of Physician Assistant status by the National Commission on Certification of Physician Assistants (NCCPA) and by the State Board of Medical Examiners or
  • Completion of an accredited Nurse Practitioner/Certified Advanced Practice Registered Nurse program with state and national certification

Licensure:

  • Possess a Physician Assistant license or an Advanced Practice Registered Nurse license in good standing in the State of Colorado

Experience:

  • 2 to 5 years’ experience as a Nurse Practitioner or Physician Assistant in primary care, internal medicine, or family medicine preferred
  • Experience with Telehealth platforms and delivering virtual care

Knowledge, Skills, and Abilities:

  • Good physical health and emotional stability are required to maintain pace and adequate interpersonal relationships
  • Ability to relate with co-workers, patients, and their sensitivity to each individual's needs
  • Must work well with other patient care team members
  • Concern for the quality of medical care and satisfaction of patients
  • Ability to make appropriate interpersonal relationships
  • Highly motivated, tech-savvy, and detail-oriented for this unique position. Reliable high-speed internet connection and a dedicated HIPAA-compliant home office workspace
  • High proficiency with electronic health Record systems
  • Excels in virtual patient care and is capable of independent decision making within a defined protocol
  • Enthusiastic about improving patient access and clinical quality
  • Ability to interpret to the patient and family the implications of a diagnosis and the nature of treatment consistent with the action and wishes of the medical staff
  • Ability to speak Spanish preferred
