- PRN NP, CHA - 8-12 Hours per week

Responsibilities
- Conduct annual risk assessments in residential or telehealth settings within the scope of practice for a Nurse Practitioner, as delegated by the Collaborative Physician.
Essential Duties and Responsibilities
- Perform comprehensive in-home health risk assessments to identify active and chronic conditions, as well as physical, mental, and social needs.
- Take patient history, examine, determine diagnoses.
- Provide documentation of patient visits per NCQA standards.
- Take vital signs as necessary.
- Place case management referrals and communicate with PCP as needed.
- Communicate with patients, caregivers, nurses, other providers, and vendors to ensure proper diagnosis.
- Observe OSHA Universal Precautions during clinical duties.
- Maintain patient confidentiality.
- Attend meetings and participate in committees as requested.
- Engage in professional development activities and maintain licenses and affiliations.

Qualifications
- Active/unrestricted medical license.
- Active CPR Certification.
- Board eligible or board certified in family medicine.
- Excellent EHR skills.
Preferred Qualifications
- Geriatric training/experience.
- Teamwork skills and ability to maintain effective working relationships.
Conditions of Role
- Adaptability to weather and home environments.
- Full range of motion, handling/lifting patients, manual dexterity, visual and auditory acuity, standing, bending, walking, stair climbing.
- Ability to lift up to 50 pounds.
- Ability to drive up to 150 miles daily in urban/rural settings.
Pay Rates
- Maximum: USD $145.00 per visit.
- Minimum: USD $85.00 per visit.
