Position Details

PRN NP, CHA - 8-12 Hours per week.

Responsibilities include conducting comprehensive in-home health risk assessments, taking patient history, examining, diagnosing, documenting visits, taking vital signs, placing referrals, communicating with healthcare providers, and maintaining patient confidentiality.

Qualifications

• Active/unrestricted medical license
• Active CPR Certification
• Board eligible or board certified in family medicine
• Outstanding EHR skills

Preferred Qualifications

• Geriatric training/experience
• Teamwork skills and effective working relationships

Conditions of Role

• Adaptability to weather conditions and home environments
• Physical ability to handle/lift patients up to 50 pounds and to drive up to 150 miles daily

Compensation

Max Pay Rate: USD $145.00

Min Pay Rate: USD $85.00
