Job summary

Choice Health At Home in Scottsdale, AZ is seeking a Hospice Nurse Practitioner to join our patient-focused team. The NP will conduct face-to-face visits at patients' residences, collaborate with the IDG and hospice physicians, and provide palliative care and family education.

Applicants must hold a current STATE RN license and Advanced Practice Nurse license, with strong communication skills and the ability to work in an unstructured environment while prioritizing compassionate care.

Qualifications

• Current STATE RN license and Advanced Practice Nurse (APN) license.
• Experience in hospice/palliative care preferred.
• Ability to educate patients and families.

Responsibilities

• Collaborates with the IDG to assure appropriate evaluation and palliative treatment of terminal disease.
• Consults with and educates the patient/family regarding disease process, self-care techniques, end-of-life care; provides training to other staff as needed.
• Perform Face-to-Face visits at the patient's place of residence.
• Interacts with attending and consulting physicians as necessary regarding palliative care, in collaboration with hospice physicians.
• Provides clinical assessment results to hospice physician after making face-to-face encounters with patients, as assigned.
• Keeps up with and maintains education/knowledge in pharmacology and palliative medicine.
• Knowledgeable of hospice criteria.

The Choice Hospice Nurse Practitioner is a passionate provider and hospice advocate- a proven clinician responsible for helping shepherd the care of the seriously ill. The ideal candidate will have a personality type that is predispositioned to thrive in an unstructured environment and is ok with reacting to patient needs while being a supportive team member with excellent ability to communicate with team members.
