Qualifications

• Graduate of an accredited School of Nursing and accredited Nurse Practitioner Program.
• Current nursing licensure in the State of Arizona.
• A minimum of one year of Nurse Practitioner experience and one year of hospice or palliative care experience.

Responsibilities

• Provide medical care, palliate and manage terminal illness under supervision.
• Develop individualized care plans and prescribe medications.
• Coordinate hospice care services with the interdisciplinary team.
