Job summary

Choice Hospice, Inc. seeks a Nurse Practitioner to join the Scottsdale hospice team on a PRN basis. You will provide home-based hospice care and collaborate with the IDG and physicians to ensure compassionate, evidence-based treatment for seriously ill patients.

The successful candidate will hold current RN and Advanced Practice Nurse licenses, communicate effectively with families and staff, and adapt to an unstructured environment while supporting the care team.

Qualifications

• Current RN license and Advanced Practice Nurse license required.
• Strong communication with patients, families and staff.

Responsibilities

• Collaborates with the IDG to evaluate and manage terminal disease care.
• Educates patient/family on disease process and self-care in end-of-life care; trains staff as needed.
• Performs Face-to-Face visits at patient residences.
• Coordinates with attending and consulting physicians regarding palliative care.

The Choice Hospice Nurse Practitioner is a passionate provider and hospice advocate- a proven clinician responsible for helping shepherd the care of the seriously ill. The ideal candidate will have a personality type that is predispositioned to thrive in an unstructured environment and is ok with reacting to patient needs while being a supportive team member with excellent ability to communicate with team members.

As a Hospice Nurse Practitioner , you will:

• Collaborates with the IDG in assuring appropriate evaluation and palliative treatment of terminal disease.
• Consults with and educates the patient/family regarding disease process, self-care techniques, end-of-life care. Provides training to other staff as needed.
• Perform Face-to-Face visits at the patient's place of residence.
• Interacts with attending and consulting physicians as necessary regarding palliative care, in collaboration with hospice physicians.
• Provides clinical assessment results to hospice physician after making face-to-face encounters with patients, as assigned.
• Keeps up with and maintains education/knowledge in pharmacology and palliative medicine.
• Knowledgeable of hospice criteria.
