Job summary

HarmonyCares is seeking a Nurse Practitioner for Primary Care to manage a geriatric patient panel in home-based settings. The role is Mon-Fri with no weekends or evenings, focusing on chronic condition management and patient-centered care.

You will chart during workday using Athena EMR plus AI Scribe, with a mission to deliver high-quality home health services. HarmonyCares operates across 14 states and supports clinicians with a team-based, physician-driven model.

Qualifications

• Licensed NP in the state where HarmonyCares Medical Group office is located.
• Federal DEA license/eligibility.
• Geriatric training/experience preferred.
• Active CPR/BLS certification.
• Board certification.
• Must maintain a valid driver’s license and good driving record

Responsibilities

• Provide primary care in home health/geriatric settings with focus on patient care, not volume.
• Take time to address patient’s needs and manage chronic conditions for your own panel.
