Job summary

HarmonyCares in Michigan is seeking a Geriatric Nurse Practitioner to deliver in-home primary care to high-acuity patients, Monday through Friday, with no weekends. Utilize Athena EMR + AI Scribe to complete charting during regular work hours.

You’ll join a team-based, physician-driven model focused on patient-centered care in patients' homes, with competitive compensation and comprehensive benefits.

Qualifications

• Licensed NP in the state where HarmonyCares Medical Group operates
• DEA license or eligibility
• Geriatric and home health experience preferred
• Active CPR/BLS certification
• Board certification
• Valid driver’s license with good driving record

Responsibilities

• Provide primary care to high-acuity patients in home settings, Monday–Friday
• Deliver patient-centered care with focus on value rather than volume
• Collaborate with an interdisciplinary team to manage chronic conditions

The Nurse Practitioners we seek are those who have directly related experience in home health, geriatrics, hospice and primary care. The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.
