Job summary

Impact Health PC in Poughkeepsie, NY, is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to provide comprehensive medical/primary care to residents in skilled nursing facilities.

You will assess and manage acute and chronic conditions, develop treatment plans, order tests, coordinate with physicians and care teams, and ensure compliant documentation. Flexible Monday–Friday hours, no nights or weekends, with strong clinical support and competitive earnings.

Qualifications

• As active NY NP or PA license in good standing.
• Minimum 2 years of post-certification clinical experience preferred.
• Experience in skilled nursing facilities or primary care preferred.

Responsibilities

• Provide comprehensive medical/primary care to residents in SNFs.
• Assess, diagnose and manage acute and chronic conditions.
• Develop individualized treatment plans and follow-up.
• Coordinate with physicians, nursing staff and care teams.
• Ensure accurate clinical documentation and appropriate orders.
