We are seeking a compassionate, collaborative, and clinically skilled Nurse Practitioner or Physician Assistant to join our team. This role offers the opportunity to build meaningful relationships with participants and caregivers while working alongside an experienced interdisciplinary care team dedicated to improving outcomes for frail older adults.
Position Summary
The Nurse Practitioner/Physician Assistant provides primary care services to Elderhaus PACE participants through comprehensive assessments, ongoing medical management, acute care interventions, and care coordination. This position plays a vital role in helping participants maintain their highest possible level of health, independence, and quality of life.
As a member of the interdisciplinary team, the Nurse Practitioner/Physician Assistant collaborates closely with healthcare professionals, caregivers, and community partners to develop and implement individualized plans of care that address each participant's unique needs.
