Job summary

Enhabit Home Health & Hospice seeks a Nurse Practitioner to join our hospice team. You will deliver medical care to patients in home settings, determine hospice eligibility, and manage pain and symptoms as part of an interdisciplinary team.

The ideal candidate is a graduate of an approved school, licensed as an NP in the practice state, with knowledge of Medicare requirements, and a valid driver's license. CPR certification and reliable transportation are required for this role.

Qualifications

• Must be a graduate of an approved school and licensed as an NP in the state they practice.
• Regulatory knowledge including Medicare conditions of participation.
• Basic technology skills including operation of a mobile device.
• Must possess a valid state driver license and maintain automobile liability insurance.

Responsibilities

• Provide medical care to hospice patients.
• Determine hospice eligibility and pain and symptom management.
• Participate in the care of the hospice patient as a member of the interdisciplinary team.

Responsibilities

Qualifications

Education and experience, essential

• Must be a graduate of an approved school, and licensed as a nurse practitioner (NP) in the state in which they currently practice.
• Must have demonstrated knowledge of regulatory requirements, including Medicare conditions of participation.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Education and experience, preferred

• At least five years nursing experience with a minimum of one year in hospice preferred.
• At least three years of clinical registered NP experience preferred.

Requirements

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
• Must possess CPR certification
