Job summary

Twin Cities Physicians in Minnesota seeks a Next-Level Wound Care Nurse Practitioner (AGNP/CNP) to manage complex wounds within assisted living, memory care, and long-term care settings across the Twin Cities. You'll partner with experienced teams and apply advanced clinical judgment to drive improved healing outcomes.

This full-time position offers $120,000 to $160,000+ total compensation (base + productivity incentives) with mileage reimbursement, CME allowance, malpractice coverage, DEA

Qualifications

• Active Minnesota CNP license; AGNP preferred.
• Experience in wound care, geriatrics, primary care, post-acute care or skilled nursing.
• Strong clinical judgment for medically complex patients.
• Valid driver's license and ability to travel across the Twin Cities Metro Area.

Responsibilities

• Provide advanced wound care assessments, treatment planning, and ongoing management.
• Conduct routine rounding visits across assisted living, memory care, and long-term care communities.
• Manage medications, diagnostics, treatment plans, and follow-up care.
• Collaborate with physicians, nurses, therapists, caregivers, and facility teams.
• Support chronic disease management, preventive care initiatives, and continuity of care.
• Improve healing outcomes and reduce avoidable complications and hospitalizations.
