Twin Cities Physicians is seeking a Next-Level Wound Care Nurse Practitioner to serve in community-based senior care across Minnesota. You’ll handle advanced wound care, rounding visits, and collaborative care in assisted living, memory care, and long-term care settings.

Qualifications

• Active Minnesota CNP license (AGNP preferred).
• Experience in wound care, geriatrics, primary and post-acute care.

What You’ll Be Doing

• Provide advanced wound care assessments, treatment planning, and ongoing management.

• Strong clinical judgment and confidence managing medically complex patients.
• Valid driver's license and ability to travel throughout the Twin Cities Metro Area.
