Job summary

Twin Cities Physicians is seeking a full-time Advanced Wound Care Nurse Practitioner to serve senior living communities across the Twin Cities Metro. You will provide expert wound care and clinical support directly where patients live, handling complex medical cases while supporting dignity, comfort, and quality of life.

Ideal candidates have Wound Care and geriatrics experience, an AGNP or MN-licensed CNP, and willingness to travel across the metro area.

Qualifications

• Active Minnesota Certified Nurse Practitioner license; AGNP preferred.

Responsibilities

• Provide advanced wound care assessments, treatment planning, and management of complex wounds.
• Conduct routine rounding visits in assisted living, memory care, skilled nursing, and long-term care communities.
• Manage medications, diagnostics, treatment plans, and follow-up care.
• Collaborate with physicians, nurses, therapists, caregivers, and community staff.
• Support proactive interventions to improve healing outcomes and reduce complications.
• Help patients maintain the highest possible quality of life throughout their healthcare journey.

What You'll Be Doing

Qualifications

• Active Minnesota Certified Nurse Practitioner (CNP) license (AGNP preferred).
• Experience in wound care, geriatrics, primary care, post-acute care, or skilled nursing preferred.
• Strong clinical judgment and confidence managing medically complex patient populations.
• Valid driver's license and ability to travel throughout the Twin Cities Metro Area.
