Job summary

Diagnostic Group in Nederland, TX seeks a full-time Clinic Nurse Practitioner to join our medical team. The role combines outpatient clinic duties with inpatient coverage, including weekend and holiday rotations.

The NP will assess, diagnose, and manage acute and chronic conditions while coordinating care with the healthcare team. The candidate should hold a TX APRN license with national NP certification, BLS and ideally ACLS.

Qualifications

  • Current and unrestricted APRN license in Texas.
  • National board certification as a Nurse Practitioner.
  • Current BLS certification.
  • ACLS certification preferred.

Responsibilities

  • Conduct comprehensive assessments, histories, and exams.
  • Diagnose and manage acute and chronic conditions.
  • Develop and adjust treatment plans.
  • Order and interpret labs/imaging as indicated.
  • Prescribe medications per Texas scope.
  • Provide preventive care and routine screenings.

Job description

Diagnostic Group is seeking a full-time Clinic Nurse Practitioner (NP) to join our medical team in Nederland, Texas.

This position is primarily clinic-based and provides comprehensive care to patients with acute and chronic medical conditions, as well as preventive and routine healthcare needs. While the primary focus of this position is outpatient care, inpatient coverage is also required as needed, including participation in weekend and holiday rotations and coverage for other inpatient providers when necessary. The ideal candidate will have strong clinical assessment and critical-thinking skills, excellent communication abilities, and the flexibility to provide high-quality care in both outpatient and inpatient settings.

Essential Duties and Responsibilities
Outpatient Responsibilities
  • Conduct comprehensive patient assessments, medical histories, and physical examinations.
  • Diagnose and manage acute and chronic medical conditions.
  • Develop individualized treatment and ongoing care plans.
  • Order and interpret laboratory studies, imaging, and other diagnostic testing as clinically indicated.
  • Prescribe medications and therapeutic treatments in accordance with Texas scope-of-practice requirements.
  • Provide preventive health services, routine screenings, and health maintenance.
  • Counsel patients and families regarding disease prevention, wellness, medications, lifestyle modifications, and long-term management of chronic conditions.
  • Monitor patient progress and adjust treatment plans as clinically appropriate.
  • Coordinate referrals to specialists and other healthcare services when needed.
  • Collaborate with physicians and other members of the healthcare team to ensure continuity and coordination of care.
  • Maintain timely, accurate, and complete documentation in the electronic health record (EHR).
Inpatient Responsibilities
  • Assist Hospitalists with new admissions, hospital rounds, and discharges.
  • Evaluate and assist in the management of acute and chronic medical conditions in hospitalized patients.
  • Review laboratory results, imaging, medications, and other diagnostic information to assist with ongoing patient management.
  • Monitor patient progress and response to treatment and promptly communicate significant changes in condition to the Hospitalist.
  • Collaborate with physicians, nursing staff, case management, specialists, and other members of the multidisciplinary healthcare team.
  • Assist with discharge planning, medication reconciliation, patient education, and coordination of follow-up care.
  • Facilitate effective patient handoffs and continuity of care following hospitalization.
Qualifications
Licensure & Certification
  • Current and unrestricted Advanced Practice Registered Nurse (APRN) license, or equivalent advanced practice license, in the State of Texas.
  • Current applicable national board certification as a Nurse Practitioner.
  • Current Basic Life Support (BLS) certification required.
  • Advanced Cardiovascular Life Support (ACLS) certification preferred.
Clinical Skills
  • Strong physical assessment, clinical decision-making, and critical-thinking skills.
  • Ability to diagnose and manage acute and chronic medical conditions.
  • Knowledge of preventive care, health maintenance, and chronic disease management.
  • Ability to recognize changes in patient condition and appropriately escalat[e] concerns.
  • Strong organizational, communication, and documentation skills.
  • Proficiency with electronic health records (EHR).
  • Ability to work effectively in both outpatient and inpatient clinical settings.
  • Ability to collaborate effectively with physicians and multidisciplinary healthcare teams.
  • Previous primary care, family medicine, internal medicine, outpatient, or inpatient experience preferred.
Schedule
  • Full-time position
  • Primary schedule: 8:00am - 5:00pm; Monday - Friday
  • Primary responsibilities are outpatient and clinic-based.
  • Participation in weekend and holiday inpatient rotations is required.
  • Additional inpatient coverage may be required as needed to support staffing needs, including coverage for scheduled absences or PTO.
