Mobile Medical is seeking a Nurse Practitioner (NP/APRN) to deliver primary care to patients in their homes and through telehealth in the Jacksonville area. The role emphasizes one-on-one patient interactions, a supportive provider team of over 70 clinicians, and a schedule designed to avoid high-volume clinic settings.

Qualifications

• Master's Degree from an accredited Nurse Practitioner program.
• National Board Certification as Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (AGNP), or Adult Nurse Practitioner (ANP).
• Current DEA Registration or ability to obtain.
• Professional Liability Insurance.
• Valid Florida Driver's License.
• Reliable transportation.

Responsibilities

• Provide comprehensive primary care to patients in homes and via telehealth.
• Perform in-home health assessments and manage acute/chronic conditions.
• Develop individualized treatment plans and monitor progress.
• Order, review, and interpret labs and diagnostics.
• Prescribe medications and monitor effectiveness.

COMPENSATION & EARNING POTENTIAL

Compensation is based on approximately 40 visits per week (telehealth and in-person), with additional incentives for visits above 40 per week.

There are also opportunities to increase earnings based on productivity and the mix of telehealth and in-person visits.

Average NP earnings are approximately $154,000 annually with additional visit incentives.

WHAT YOU'LL DO

As a Mobile Primary Care Nurse Practitioner, you will provide comprehensive primary care to patients in their homes and through telehealth.

Clinical Responsibilities

Perform comprehensive in-home health assessments

• Evaluate and treat acute and chronic medical conditions
• Develop individualized treatment and care plans
• Order, review, and interpret laboratory and diagnostic testing
• Prescribe medications and monitor treatment effectiveness
• Provide preventive care and health education
• Educate patients and caregivers on disease management
• Coordinate care with physicians, specialists, home health agencies, and other healthcare professionals
• Maintain accurate and timely electronic medical record documentation
• Participate in population health and quality initiatives
• Build trusted, long-term relationships with patients and caregivers

WHAT YOU BRING

Required

• Master\'s Degree from an accredited Nurse Practitioner program
• National Board Certification as one of the following:
