Job summary

Twin Cities Physicians is seeking a Wound Care Nurse Practitioner (AGNP/CNP) to provide on-site care across assisted living, memory care, skilled nursing, and long-term care communities in the Twin Cities Metro. You’ll manage complex wounds and support chronic disease management with a patient-centered approach.

Qualifications

• Active Minnesota Certified Nurse Practitioner (CNP) license; AGNP preferred.
• Experience in wound care, geriatrics, primary care, or post-acute care settings preferred.
• Strong clinical judgment and ability to manage medically complex patients.
• Valid driver’s license and ability to travel throughout the Twin Cities Metro Area.

Responsibilities

• Provide advanced wound care assessments, treatment planning, and ongoing management.
• Conduct routine rounding visits across assisted living, memory care, skilled nursing, and long-term care communities.
• Manage medications, diagnostics, treatment plans, and follow-up care.
• Collaborate with physicians, nurses, therapists, caregivers, and facility staff.
• Support chronic disease management, preventive care initiatives, and continuity of care.
• Help reduce avoidable hospitalizations and improve patient outcomes.

Compensation: $120,000 to $160,000+ Total Compensation (Base Salary + Productivity Incentive Program) + Mileage Reimbursement
