Job summary

Rockford Nephrology Associates in Rockford, IL, is seeking a Nurse Practitioner to join our nephrology team. You’ll provide patient-centered kidney care across outpatient, hospital, and dialysis settings, collaborating with physicians to manage chronic kidney disease and related conditions.

Qualifications

• Graduate degree from an accredited Nurse Practitioner program.
• Current Illinois licensure and applicable national certification.
• At least two years of bedside nursing experience is required, preferably in ICU/stepdown/ED or similar.
• Strong clinical assessment, diagnostic, and treatment-planning skills.
• Excellent communication and interpersonal skills.
• Ability to work collaboratively with physicians and multidisciplinary healthcare teams.
• Nephrology experience is preferred but not required.

Responsibilities

• Evaluate patients, perform comprehensive histories and physical exams, and develop individualized treatment plans.
• Manage patients with chronic kidney disease in outpatient settings.
• Monitor patients on hemodialysis and peritoneal dialysis and address complications.
• Participate in inpatient nephrology care including acute and chronic kidney conditions.
• Order and interpret labs and studies and manage medications within scope.
• Educate patients and families on kidney health and disease management.
• Coordinate care across outpatient clinics, hospitals, and dialysis centers.
• Collaborate with physicians and multidisciplinary teams to promote outcomes and continuity of care.

About the Role

As a Nurse Practitioner with RNA, you’ll provide comprehensive, patient-centered care to individuals with kidney disease across multiple clinical settings. You’ll have the opportunity to develop meaningful patient relationships, collaborate with physicians, and help patients manage complex conditions throughout their care journey.

Key Responsibilities:

• Evaluate patients, perform comprehensive histories and physical examinations, and develop individualized treatment plans.
• Manage patients with chronic kidney disease and other kidney-related conditions in the outpatient setting.
• Monitor patients receiving hemodialysis and peritoneal dialysis, addressing treatment needs and potential complications.
• Participate in inpatient nephrology care, including the management of acute and chronic kidney conditions.
• Order and interpret laboratory and diagnostic studies and manage medications within your scope of practice.
• Educate patients and families on kidney health, treatment options, and long-term disease management.
• Coordinate care across outpatient clinics, hospitals, dialysis centers, and other healthcare providers.
• Collaborate with physicians and multidisciplinary teams to promote positive patient outcomes and continuity of care.

What We’re Looking For

• Graduate degree from an accredited Nurse Practitioner program or Physician Assistant Studies program.
• Current Illinois licensure and applicable national certification.
• At least two years of bedside nursing experience is required, preferably in an ICU, stepdown, emergency department, cardiology, or similar clinical setting.
• Strong clinical assessment, diagnostic, and treatment-planning skills.
• Excellent communication and interpersonal skills.
• Ability to work collaboratively with physicians and multidisciplinary healthcare teams.
• A commitment to compassionate, patient-centered care.

Nephrology experience is preferred but not required. Candidates with relevant experience in critical care, hospital medicine, dialysis, or other acute care specialties are encouraged to apply.
