Job summary

Stat Home Health is seeking a dedicated medical professional to provide primary care services across Louisiana and Mississippi. The role involves conducting in-home visits, telehealth consultations, and managing chronic conditions, all while promoting preventive care and continuity.

Qualifications

• DEA license required.
• Experience in home health, hospice, or facility-based care required.
• Comfortable working independently across diverse care environments.

Responsibilities

• Provide primary medical care services in home, facility, and telehealth settings.
• Conduct physical exams, wellness visits, and manage chronic and acute conditions.
• Order and interpret diagnostic tests and labs.
• Prescribe and manage medications and individualized care plans.
• Coordinate care with interdisciplinary teams and community partners.
• Promote preventive care, patient education, and continuity of care.
• Participate in after-hours or on-call rotation as needed.
