Job summary

The Carpenter Health Network is seeking a compassionate PRN Nurse Practitioner in Lafayette, LA, to provide comprehensive primary care in home, assisted living and nursing facilities, and via telehealth. As part of our team, you’ll conduct physical exams, manage medications, and coordinate care across diverse settings. The ideal candidate will hold an active Nurse Practitioner license in Louisiana and have experience in home health or facility-based care.

Qualifications

• Active Nurse Practitioner license in Louisiana required.
• DEA license is mandatory.
• Experience in home health, hospice, or facility-based care is necessary.

Responsibilities

• Provide primary medical care services in home, facility, and telehealth settings.
• Conduct physical exams, wellness visits, and manage chronic and acute conditions.
• Order and interpret diagnostic tests and labs.
• Prescribe and manage medications and individualized care plans.
• Coordinate care with interdisciplinary teams and community partners.
• Promote preventive care, patient education, and continuity of care.
