Job summary

Homedica House Calls in Monroe, LA is seeking a Full-Time Nurse Practitioner to provide high-quality in-home medical care, telehealth visits, and post-acute facility services.

You will conduct exams, manage chronic and acute conditions, order tests, prescribe medications, and coordinate care with interdisciplinary teams, while benefiting from ESOP ownership and comprehensive health, retirement, and time-off benefits.

Qualifications

• Active Nurse Practitioner license in Louisiana.
• DEA license required.
• Experience in home health, hospice, or facility-based care.

Responsibilities

• Provide primary medical care services in home, facility, and telehealth settings.
• Conduct physical exams, wellness visits, and manage chronic and acute conditions.
• Order and interpret diagnostic tests and labs.
• Prescribe and manage medications and individualized care plans.
• Coordinate care with interdisciplinary teams and community partners.
• Promote preventive care, patient education, and continuity of care.
• Participate in after-hours or on-call rotation as needed.
