Job summary

Allure Healthcare Services in the Knox County Area, Illinois, is seeking a Nurse Practitioner to lead internal medicine services at a Skilled Nursing Facility. You will collaborate with the interdisciplinary team and physicians, serving as a day-to-day clinical leader while overseeing care for all residents.

You will perform comprehensive health assessments, manage acute and chronic illnesses, document with CPT coding, and guide discharge planning with support from medical directors and nursing

Qualifications

• Active APN licensure with an applicable state license.
• Minimum one year SNF experience preferred.
• Strong communication and leadership skills.

Responsibilities

• Provide comprehensive health assessments and prescribe treatments per facility protocol.
• Lead day-to-day clinical operations and collaborate with physicians and nursing staff.
• Coordinate admissions, discharges, and monthly stability visits for residents.
• Document thoroughly and CPT coding for all visits.

As the Nurse Practitioner you will act as a Skilled Nursing Facility's leading provider of internal medicine services. You will collaborate with the facility's interdisciplinary team and physicians, serving as a day-to-day clinical leader while helping oversee the care of all of the facility's residents.

You will work with the entire facility's patient population - short and long-term residents - overseeing their day-to-day care with the assistance of the facility's nursing staff and guidance from the facility's medical directors and attending physicians.

As a Nurse Practitioner, you will provide types of care, including but not limited to:

• Be comfortable and proficient with documentation and CPT coding
• Evaluation of new-admission visits
• Coordination of discharge visits
• Monthly stability visits for long term residents
• Evaluations for patients exhibiting changes in condition
• Advanced Care Planning

Other Duties & Responsibilities

• Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease
• Prescribe all necessary medications and treatments
• Perform comprehensive physical examinations of patients
• Develop and implement patient management policies and procedures
• Facilitate referrals to other healthcare professionals and medical facilities
• Communicate with attending physicians and/or specialists regarding patient care
• Be comfortable and proficient with documentation and CPT coding

Requirements

Licensure / Certification / Registration:

• Advanced Practice Licensure (APN) with an applicable state license
• Must have an active license and DEA #
• Previous experience in nursing or other medical fields
• Minimum one-year experience in a Skilled Nursing setting (preferred)
• Ability to build rapport with patients and their loved ones
• Ability to thrive in a fast-paced environment
• Excellent written and verbal communication skills
• Strong leadership experience with emphasis on teamwork in a healthcare setting
