Qualifications

  • Current Texas RN and APRN licensure.
  • Current national Nurse Practitioner certification appropriate to the assigned patient population.
  • Family NP or PMHNP background, or NP experience in PM&R/rehabilitation.
  • Comfort and willingness to work directly with adult patients who exhibit behavioral challenges.
  • Strong clinical assessment, communication, and interdisciplinary collaboration skills.
  • Experience caring for adults with brain injury, neurological conditions, psychiatric disorders, or complex rehabilitation needs.
  • Current BLS certification and prescribing credentials for assigned duties.

Responsibilities

  • Perform comprehensive assessments of adult patients, including medical histories and cognitive/behavioral concerns.
  • Develop and adjust individualized treatment plans with physicians and the interdisciplinary team.
  • Manage acute and chronic conditions within the provider's specialty and scope.
  • Order and interpret diagnostic testing, prescribe medications, and monitor treatment effects.
  • Evaluate changes in patient condition and coordinate timely treatment or transfer.
  • Collaborate with nursing, rehabilitation, behavioral health, and case management teams.
  • Participate in rounds and care conferences to align medical and rehabilitation goals.
  • Educate patients, families, and caregivers about health conditions and plans.
  • Maintain timely documentation and contribute to patient safety and quality improvement.

Nexus Neurorecovery Center - Conroe is seeking a Nurse Practitioner to support ouradult patient population with complex medical, neurological, and behavioral needs. We welcome Family Nurse Practitioners, Psychiatric-Mental Health Nurse Practitioners, and Nurse Practitioners with experience in Physical Medicine & Rehabilitation.

Candidates must be comfortable caring for adults with behavioral challenges and collaborating with an interdisciplinary team to address the connection between physical health, cognition, behavior, and recovery.

Located on a26-acre residential rehabilitation campus, Nexus Neurorecovery Center - Conroe provides a community-oriented environment where patients can work toward meaningful recovery. The center provides post-acute rehabilitation and ongoing care for individuals recovering fromtraumatic brain injury, stroke, spinal cord injury, and other neurological conditions. Its programs include:

  • Rise Program: Rehabilitation and ongoing medical care following serious neurological injuries and conditions.
  • Life Care Program: Ongoing, lifelong medical care for individuals with lasting needs following catastrophic injury.

The Nurse Practitioner provides assessment, treatment, and ongoing clinical management for adult patients in collaboration with physicians and the interdisciplinary care team. This role supports individuals whose neurological conditions may be accompanied by cognitive impairment, behavioral dysregulation, psychiatric symptoms, and complex medical needs.

The ideal candidate brings strong clinical judgment, patience, and a practical, compassionate approach to caring for adults who may have difficulty communicating needs, regulating emotions, or participating consistently in treatment. Assigned responsibilities will align with the provider’s education, certification, clinical privileges, and scope of practice.

Key Responsibilities
  • Perform comprehensive assessments of adult patients, including medical histories, physical examinations, and evaluation of relevant cognitive, functional, and behavioral concerns.
  • Develop and adjust individualized treatment plans in collaboration with physicians and the interdisciplinary team.
  • Manage acute and chronic conditions within the provider’s specialty and scope of practice.
  • Order and interpret diagnostic testing, prescribe medications, and monitor treatment effectiveness and adverse effects as appropriate.
  • Evaluate changes in patient condition and coordinate timely treatment, specialty consultation, or transfer to a higher level of care when indicated.
  • Help identify medical concerns that may contribute to behavioral changes, including pain, illness, medication effects, and sleep disturbances.
  • Collaborate with nursing, rehabilitation, behavioral health, and case management professionals to support consistent approaches to patient care.
  • Participate in rounds and care conferences, connecting clinical treatment with rehabilitation goals and everyday functioning.
  • Educate adult patients, families, and caregivers about health conditions, medications, treatment plans, and ongoing care needs.
  • Maintain timely, accurate documentation and contribute to patient safety and quality improvement initiatives.
