Job summary

Palm Medical Centers is hiring a Primary Care Nurse Practitioner to join our senior primary care team in Florida. You will deliver value-based primary care, focusing on prevention, patient education, and comprehensive management of chronic conditions.

Qualifications

• 2-3 years’ experience in primary care/value-based care settings (Medicare/Medicaid)
• Active Florida APRN license
• DEA license or eligibility within 2 months of hire
• Strong chronic disease management and preventive care knowledge
• Excellent documentation and patient relationship skills
• Proficient with EMR systems (eCW a plus)

Responsibilities

• Provide primary care in a value-based setting.
• Collaborate with care team to coordinate patient care.
• Maintain accurate and timely documentation in the EMR.

Job Requirements:

• Have at least 2-3 years’ experience in the primary care/value-based care setting, working with Medicare Advantage, Medicare, and Medicaid patient populations
• Must have an active and unrestrictive Florida APRN license
• Must have DEA license or eligible to obtain it within 2 months of date of hire
• Strong understanding of chronic disease management, preventative care, and quality metrics in a value-based care model
• Excellent documentation and patient relationship-building skills
• Proficient with EMR systems (eCW a plus!)
