Job summary

CareMed Health Solutions in Lexington, KY is seeking licensed Nurse Practitioners to provide medical services to residents in long-term care facilities. You will deliver primary and preventive care within an interdisciplinary team to improve life and wellbeing of elderly residents.

Responsibilities include managing acute changes, advanced care planning, chronic disease management, prescribing medications per Kentucky law, ordering tests, and coordinating with facility staff and primary care

Qualifications

• Current, unrestricted NP license in Kentucky.
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team to coordinate patient care.
• Basic coding knowledge preferred, not required.

Responsibilities

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.
• Manage Residents with acute change in condition.
• Advanced care planning and chronic care management.
• Prescribe appropriate medications, including controlled substances in accordance with KY Law.
• Order, review and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition and the care plan.
• Collaborate with primary care physician.
