Job summary

CareMed Health Solutions is seeking a Nurse Practitioner to deliver primary and preventive care to residents in skilled nursing facilities in Kentucky.

You will manage acute changes, participate in advanced care planning, and coordinate with primary care physicians within an interdisciplinary team. KY licensure, board certification, and DEA are required, with EMR experience preferred.

Qualifications

• Current, unrestricted NP license in Kentucky.
• Board certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience in long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team.
• Psych experience a plus.

Responsibilities

• Provide primary and preventative care for long term care residents in skilled nursing facilities.
• Manage residents with acute changes in condition.
• Advanced care planning and chronic care management.
• Prescribe medications, including controlled substances per KY law.
• Order, review and follow up on treatments and diagnostics.
• Communicate with nursing home staff about condition changes and care plan.
• Collaborate with primary care physicians.
