Job summary

Oakmedgroup is seeking an Advanced Practice Provider to deliver diagnostic and therapeutic care in Skilled Nursing, Memory Care, and related environments. The APP works under physician supervision to perform comprehensive evaluations, manage acute and chronic conditions, and document care plans for adults and geriatric patients.

Qualifications include an advanced nursing degree, NP license in Wisconsin, and relevant certifications.

Qualifications

  • Master of Science - Nursing or higher required.
  • Current Wisconsin Advanced Practice Nurse Practitioner license or eligible.
  • ANCC/AANP/NCCPA certification and BLS valid.
  • Experience in Internal Medicine preferred.
  • Ability to work under physician supervision and in fast-moving settings.

Responsibilities

  • Provide general medical care to adults/geriatric patients.
  • Perform evaluations including history and physical exams.
  • Address acute and chronic primary care complaints and assess urgency.
  • Create and implement medical plans of care.
  • Prescribe medications as appropriate and document care.

JOB SUMMARY

The Advanced Practice Provider (APP) functions under the clinical supervision of licensed physicians, provides diagnostic and therapeutic health care to patients in Skilled Nursing, Memory Care, Assisted Living, Independent Living, Inpatient Rehabilitation Hospitals, and Inpatient Psychiatric Hospital environments.

ESSENTIAL DUTIES and RESPONSIBILITIES
  • Provide general medical care to various and/or specific patient levels - adults/geriatric.
  • Perform comprehensive evaluations including history and physical exams.
  • Address both acute and chronic primary care complaints, and be able to ascertain medical urgency.
  • Establish and document reasonable medical diagnoses.
  • Order pertinent diagnostic laboratory and radiology testing for the medical diagnosis or presenting symptom.
  • Apply principles of critical thinking to a variety of practical and emergent situations and accurately follow standardized procedures that may call for deviations.
  • Accept feedback and direction from the supervising physician.
  • Create and implement a medical plan of care.
  • Prescribe medications as appropriate.
  • Perform timely documentation and billing.
  • Maintain confidentiality of all patient records.

MINIMUM QUALIFICATIONS
  • Preferred one year experience in Internal Medicine
  • Excellent clinical skills, communication skills, and a strong commitment to providing excellent care that is team oriented.
  • Problem‑solves and uses resources well independently and as part of a team.
  • Willingness to work in a fast‑moving, high‑growth work setting.
  • Knowledge of Electronic Health Record (EHR), e‑prescribing software, scheduling software, and telemedicine software.
EDUCATION
  • Master of Science - Nursing
  • Doctorate of Nurse Practice
  • Bachelor of Health Science in Physician Assistant studies
LICENSE/CERTIFICATION
  • Valid Wisconsin Advanced Practice Nurse Practitioner License or
  • Certification by the American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners (AANP)
  • National Commission of Certification of Physician Assistants (NCCPA)
  • Basic Life Support for the Healthcare Provider (BLS)
PHYSICAL

While performing the duties of this job, the employee is regularly required to talk, hear and listen. The employee frequently is required to stand, walk and sit. The employee must occasionally lift and/or move up to 50 pounds. Position may require exertion up to 50 pounds of force occasionally and/or up to 10 pounds of force frequently or constantly lift, carry, push, pull or otherwise move objects.
