Job summary

Qualifications

• Current and valid State of Wyoming RN license is required.
• Bachelor’s degree in Nursing and Wyoming Advanced Practice Nursing license; Master’s degree preferred.
• Board Certification from a nationally recognized certifying body for Nurse Practitioners required.

Responsibilities

• Performs in-home history and physical assessments for members, based on referral visit type.
• Demonstrates knowledge of etiologies, risk factors, and presentation of acute and chronic conditions.
• Reviews member records prior to visits to support accurate diagnoses and plans.
